Job summary

Sung and Ward, Inc. is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our nephrology team in a full-time outpatient setting.

You will care for patients across clinics, dialysis units and affiliated hospitals, traveling throughout Westmoreland and Indiana Counties to deliver high-quality kidney care. Responsibilities include evaluating and managing CKD, coordinating dialysis care, educating patients, and maintaining accurate EMR documentation while collaborating

Qualifications

  • Current certification and licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) or eligibility for licensure.
  • Strong clinical assessment, diagnostic and patient management skills.
  • Excellent communication, documentation, and patient education abilities.
  • Ability to establish trust with patients and families.
  • Willingness to travel between offices, dialysis centers and hospitals.

Responsibilities

  • Evaluate, diagnose and manage patients with chronic kidney disease, AKI, hypertension and electrolyte disorders.
  • Conduct follow-up visits in outpatient nephrology clinics.
  • Round on dialysis patients and coordinate care with dialysis staff.
  • Participate in inpatient nephrology consultations at affiliated hospitals.
  • Manage a high-volume caseload while maintaining quality of care.
  • Order, interpret and manage diagnostic tests and labs.
  • Develop and implement individualized treatment plans with supervising physicians.
  • Educate patients and families about kidney disease, dialysis options, and medications.
  • Maintain accurate documentation in the electronic medical record.
  • Collaborate with physicians, nurses and other healthcare professionals.
  • Foster compassionate, patient-centered care and trust with families.

Position Summary

Sung and Ward, Inc. is seeking a motivated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing nephrology team. This full-time position provides comprehensive nephrology care in outpatient clinics, dialysis centers, and hospital settings. The successful candidate will play an important role in guiding patients through their healthcare journey, providing education, support, and high-quality medical care while working closely with physicians and interdisciplinary team members.

This is a dynamic role with a high patient volume and a variety of clinical experiences. The position includes travel between outpatient offices, dialysis units, and hospitals throughout Westmoreland and Indiana Counties, offering the opportunity to care for patients across the full continuum of kidney disease management.

Responsibilities
  • Evaluate, diagnose, and manage patients with chronic kidney disease, acute kidney injury, hypertension, electrolyte disorders, and other nephrology-related conditions
  • Conduct follow-up visits in outpatient nephrology clinics
  • Round on dialysis patients and coordinate care with dialysis center staff
  • Participate in inpatient nephrology consultations at affiliated hospitals throughout Westmoreland and Indiana Counties
  • Manage a high-volume patient caseload while maintaining exceptional quality of care
  • Order, interpret, and manage diagnostic tests and laboratory results
  • Develop and implement individualized treatment plans in collaboration with supervising physicians
  • Educate patients and families regarding kidney disease, dialysis options, medications, nutrition, and disease management
  • Maintain accurate and timely documentation within the electronic medical record
  • Collaborate with physicians, nurses, dialysis personnel, and other healthcare professionals to ensure continuity of care
  • Foster trusting relationships with patients and families through compassionate communication and patient-centered care
Qualifications
Required:
  • Current certification and licensure (or eligibility for licensure) as a:
  • Strong clinical assessment, diagnostic, and patient management skills
  • Excellent communication, documentation, and patient education abilities
  • Ability to establish trusting, long-term relationships with patients and families
  • Commitment to providing equitable, culturally sensitive, and patient-centered care
  • Ability to work effectively in a collaborative, fast-paced, high-volume clinical environment
  • Reliable transportation and willingness to travel between offices, dialysis centers, and hospitals
Preferred:
  • Previous experience in nephrology, internal/hospital medicine, or dialysis care
  • Familiarity with chronic disease management and care coordination
  • Critical Care experience as bedside RN if new NP grad
