Job Description

The Nurse Practitioner/Physician Assistant will work closely with practice physicians to manage nephrology and endocrinology patients. This board-certified independent practitioner is responsible for comprehensive patient care in accordance with state and federal regulations, including patient assessments, diagnoses, treatment plans, patient education, and care coordination.

This position may involve dialysis rounding, chronic kidney disease (CKD) clinic support, hospital rounding, and on-call coverage.

Responsibilities
  • Perform 50% hospital patient rounding within assigned regional pod
  • Manage 50% office visits (15–20 patients per day)
  • Support chronic kidney disease clinics and on-call coverage as scheduled
  • Collaborate with physicians and care teams to deliver patient-centered care
  • Complete accurate documentation in Electronic Health Records (EHR)
  • Participate in weekend coverage rotations, potentially in alternate pods (up to 1-hour commute)
  • Eligible for mileage reimbursement for hospital rounds exceeding clinic commute
Minimum Education and Experience Qualifications
  • Bachelor’s degree and completion of an accredited Nurse Practitioner or Physician Assistant program
  • National certification with NCCPA or AANPCB
  • Current Pennsylvania NP or PA license in good standing
  • Active DEA license
  • Minimum of 2 years’ clinical experience in a medical office, hospital, or other medical setting
  • Strong clinical knowledge, patient assessment skills, and proficiency with EHR systems
