As an Advanced Practice Provider, you'll play an integral role in our inpatient and outpatient care teams by providing high-quality, cost-effective care to patients. We are seeking a Nurse Practitioner OR Physician Assistant to:

1. Perform annual physical exams and manage the health maintenance needs of our family medicine population.
2. Care for the acute and chronic needs of patients from newborn to elderly.
3. Perform telemedicine (video and telephone) visits.
4. Perform routine clinical procedures independently including wart removal, Pap smears, IUD removal, IUD / Nexplanon placement, and skin lesion removals.

Education & Licensure

• For Nurse Practitioner: Master's or Doctorate degree from an accredited Nursing Program.
• For Physician Assistant: Bachelor's or Master's degree from an accredited PA Program.
• Minimum licensure: Active NP or PA licensure in Wisconsin.

Certifications

• Active RN and APRN certifications and licenses, including prescriptive authority, in the state of practice.
• Active board certification as a Nurse Practitioner (e.g., Family) or Physician Assistant in the area of clinical practice.
• Active DEA license or ability to obtain one prior to or upon hire.
