Work Schedule

80% FTE, Monday - Friday between 8:00 AM - 5:00 PM. No holidays or call. Hours may vary based on department needs.

As an Advanced Practice Provider, you'll play an integral role in our inpatient and ambulatory care teams by providing high-quality, cost-effective care to patients.

We are seeking a Nurse Practitioner OR Physician Assistant to:

  • Perform annual physical exams and manage health maintenance needs of our family medicine population.
  • Care for the acute and chronic needs of patients from newborn to elderly.
  • Perform telemedicine (video and telephone) visits.
  • Perform routine clinical procedures independently, including wart removal, Pap smears, IUD removal, IUD/Nexplanon placement, and skin lesion removals.
Education

If Nurse Practitioner: Master's or Doctorate degree from an accredited Nursing Program.

If Physician Assistant: Bachelor's or Master's degree from an accredited PA Program.

Work Experience

Preferred - Two (2) years of relevant clinical experience.

Licenses and Certifications

Minimum - State of Wisconsin NP or PA licensure.

  • Active RN and APRN certifications and licenses, including prescriptive authority, in practice state(s) AND
  • Active board certification as an NP in the area of clinical practice (e.g., Family).

If Physician Assistant:

  • Active PA license, including prescriptive authority, in practice state(s) AND
  • Active PA board certification.

All applicants must have an active DEA license or the ability to apply for one prior to or upon hire.
