POSITION SUMMARY:
The role of the Nurse Practitioner or Physician Assistant in a community setting is a response to society’s demand for accessible, quality health care for all populations, regardless of socio-economic status. Individual accountabilities and work volume will be established through the development of annual Success Objectives within the outlined framework.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
- Facilitates patient entry into the healthcare system by assessing primary health care needs, including eliciting comprehensive health histories, performing physical examinations, ordering and/or performing diagnostic tests, analyzing data to determine health status, and formulating diagnoses.
- Provides coordinated, continuous, and culturally competent primary healthcare through mutual goal setting and care planning.
- Prescribes various treatment modalities to address health and illness needs.
- Collaborates with community health nurses and other providers to ensure health education and counseling.
- Meets monthly productivity standards, including the number of patient visits.
- Follows role assignments aligned with Patient-Centered Medical Home standards and participates in related initiatives.
- Manages and evaluates care plans, modifying them as necessary.
- Adheres to collaborative practice principles, consulting with team members and following clinical standards.
- Obtains hospital privileges at designated hospitals as directed.
- Collaborates with multidisciplinary teams and community agencies to ensure high-quality care.
- Coordinates services with team members to meet patient needs and ensure continuity of care.
- Maintains clinical competence through mentorship, developing a patient panel, and participating in ongoing education.
- Practices within legal and ethical guidelines and documents client records accurately and comprehensively.

QUALIFICATIONS:
- Graduation from an accredited APN or PA program with successful residency completion.
- 2-3 years of clinical experience.
- Effective working relationships with colleagues, patients, and the public.
- Knowledge of medicine principles and community health center operations.
- Strong communication skills, including reading, interpreting complex documents, and engaging with patients.
- Mathematical proficiency for practical applications.
- Ability to define problems, analyze data, and interpret technical instructions.
- Active Illinois Nurse Practitioner or Physician Assistant license, DEA registration, board certification (within two years), and medical degree documentation.
PHYSICAL DEMANDS:
- Frequent standing, walking, sitting; use of hands; reaching; talking/hearing.
- Occasional lifting/moving up to 25 pounds.
