The NP/PA provides patient care, conducts physical exams, completes health history summaries, documents patients' progress, and assists with patient care management under the direct supervision of the Medical Director.

Job Functions

• Obtains and reviews patient's medical history relevant to patient care
• Conducts physical examinations
• Orders and reviews laboratory studies and diagnostic procedures
• Prescribes FDA-approved appetite suppressants according to state and federal guidelines
• Reviews patient's progress and manages care accordingly
• Maintains all required documentation for patient care and medication management
• Conducts patient consultations and provides preventive medicine counseling
• Provides training to clinical and non-clinical staff as needed
• Documents patient progress notes in the patient chart and/or EMR
• Performs other clinical duties to support the team
• Conducts follow-up calls to discuss patient progress and address concerns

Requirements

• Valid IL Medical License
• Valid IL DEA License (candidates without an active DEA License but willing to obtain one are encouraged to apply)
• Current CAQH profile (Preferred)
• Credentialed provider (Preferred)

• Weekend availability (Saturday morning/early afternoon)
