How you will make an impact:
- Facilitate patient entry into the healthcare system by assessing primary health care needs, eliciting health histories, performing physical exams, ordering diagnostic tests, analyzing data, and formulating diagnoses.
- Provide coordinated, continuous, and culturally competent primary health care through mutual goal setting and care planning.
- Adhere to collaborative practice principles, consulting with team members and following clinical standards.
- Obtain hospital privileges at designated hospitals as directed by the Chief Clinical Officer to ensure continuity of care.
- Practice within legal and ethical guidelines established by relevant authorities.
- Document accurate and comprehensive data on patient records and reports according to organizational standards.
- Follow professional standards set by governmental bodies and professional organizations.
What you will bring to Howard Brown Health
Minimum Qualifications:
- Graduation from an accredited advanced practice nursing or physician assistant program, with successful completion of a residency.
- At least 2 to 3 years of clinical experience.
- Active Illinois Nurse Practitioner or Physician Assistant license, current DEA registration, and board certification (completed within two years of employment).
