Job summary

Chestnut Health Company in Tennessee is seeking a compassionate Nurse Practitioner to join our physiatry team serving Skilled Nursing Facility patients. New graduates are encouraged to apply; you will collaborate with therapists and nurses to design and implement rehab plans.

You will perform assessments, monitor progress, document in EMRs, educate patients and families, and participate in multidisciplinary meetings.

Qualifications

• Active NP license in the state of employment.
• Current DEA license required on start date.
• Current controlled substance license required on start date.
• ANCC or AANP certification.
• Interest in Physiatry/PM&R.
• Experience in orthopedics or SNF is a plus.

Responsibilities

• Collaborate with therapists, nurses, and other professionals to develop comprehensive rehab plans.
• Conduct patient assessments, histories, exams, and diagnostic evaluations.
• Monitor progress and adjust treatment plans to improve function and independence.
• Participate in multidisciplinary meetings and educational activities.
• Document evaluations, treatment plans, progress, and discharge summaries in EMR.
• Educate patients and families about rehabilitation and post-discharge care.
