Qualifications

• Active licensure to practice in the state of employment.
• Current Controlled Substance License is required at start date.
• Current DEA license is required at start date.
• ANCC or AANP certification.
• Interest in Physiatry/PM&R and rehab care.
• SNF experience is a plus.

Responsibilities

• Collaborate with therapists, nurses and others to develop rehab plans.
• Assess patients: histories, exams, and evaluations to set goals.
• Monitor progress and adjust plans to improve outcomes and independence.
• Join multidisciplinary meetings to enhance clinical knowledge.
• Document evaluations, plans, progress, and discharge notes in EMR.
• Educate patients and families about rehab process and post-discharge care.
