Qualifications

• Active NP license in the state of employment.
• DEA license required on start date.
• Certified Nurse Practitioner certification required.
• ANCC or AANP certification preferred.
• Interest in Physiatry/PM&R and SNF experience beneficial.

Responsibilities

• Develop and implement comprehensive rehabilitation treatment plans with therapists and nurses.
• Perform patient assessments, medical histories, exams, and diagnostic evaluations.
• Monitor progress and adjust plans to improve functional outcomes and independence.
• Participate in multidisciplinary meetings and educational activities.
• Document initial evaluations, treatment plans, progress notes, and discharge summaries in EMR.
• Educate patients and families about the rehabilitation process and post-discharge care.
