Job summary

Rochester Regional Health is seeking a Nurse Practitioner to deliver remote palliative care services. You will respond to triage via video visits, educate and support patients and families, and coordinate care with the palliative team. A strong emphasis on compassion and clear documentation is required.

Position is per diem, remote, with days schedule, and requires NY NP RN certifications. Prior palliative care experience is preferred, and the department is Palliative Care.

RESPONSIBILITIES

• Patient Care. Provide primarily remote care to palliative care patients, responding to triage needs and more urgent concerns through video visits.
• Patient Satisfaction. Communicate with patients and families with empathy and understanding, providing education, support, and guidance related to their care.
• Care Coordination. Collaborate with the palliative care team to assess patient needs, provide appropriate recommendations, and coordinate care.
• In-Person Coverage. Complete required in-person training and provide occasional in-person coverage for vacation and sick-call needs.

• NP - Nurse Practitioner Certification - New York State Education Department (NYSED)
• RN - Registered Nurse - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting. PAY RANGE

PAY RANGE: $65.00 - $65.00 CITY: Rochester POSTAL CODE: 14625 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
