Job summary

Rochester Regional Health is seeking a Nurse Practitioner to deliver remote palliative care services. You will respond to triage via video visits, educate and support patients and families, and coordinate care with the palliative team. A strong emphasis on compassion and clear documentation is required.

Position is per diem, remote, with days schedule, and requires NY NP RN certifications. Prior palliative care experience is preferred, and the department is Palliative Care.

Responsibilities

• Provide remote patient care to palliative patients via video visits.
• Communicate with patients and families with empathy, offering education and guidance.
• Collaborate with the palliative care team to assess needs and coordinate care.
• Complete in-person training when required and provide occasional on-site coverage.
