Northeast Healthcare Recruitment, Inc. (NEHCR) is seeking a Nurse Practitioner or Physician Assistant for a skilled nursing facility in Ohio.

This independent, full-time role builds your own patient panel and delivers primary and episodic care with long-term relationships to residents and families. You'll see roughly 16–20 patients daily, with 6-hour rounds (7–10 a.m.), no on-call or overnights, and chart from home.

Qualifications

• Board certification required (e.g., AGNP, FNP for Nurse Practitioners; PA-C for Physician Assistants) — active and unrestricted.
• Valid, unrestricted Nurse Practitioner or Physician Assistant license in the state of operation.
• Master's degree in Nursing (Nurse Practitioner) or from an accredited Physician Assistant program — required for the credential.

Responsibilities

• See roughly 16–20 patients a day, mix of long-term chronic care and acute issues.
• Annual wellness visits and H&Ps, manage chronic/co-morbid conditions, respond to acute changes.
• Review meds and orders; coordinate with collaborating physicians, specialists, and facility staff to keep care seamless.
• Stay in touch with families and join care conferences where it makes sense.

About the Role

We're looking for an experienced, autonomous Nurse Practitioner or Physician Assistant to join our clinical team full time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity — you'll manage your own panel and build long-term relationships with patients and their families.
