Northeast Healthcare Recruitment, Inc. (NEHCR) seeks an experienced Nurse Practitioner or Physician Assistant for independent care in skilled nursing facilities. You will manage your own panel, perform wellness visits, and address acute issues in a collaborative, high-quality care setting.

Six-hour shifts, no on-call, chart from home, and strong earning potential with productivity-based compensation. Excellent benefits and CME support are included.

Qualifications

• Board certification required (e.g., AGNP, FNP; active and unrestricted).
• Valid, unrestricted NP/PA license in the operation state.
• Master's degree in Nursing or from an accredited PA program.
• Skilled nursing or long-term care experience strongly preferred.
• Strong independent clinical judgment and ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16–20 patients per day, including wellness visits and acute care.
• Manage chronic and co-morbid conditions and review meds and orders.
• Coordinate with collaborating physicians, specialists, and facility staff for seamless care.
• Stay in touch with families and participate in care conferences when appropriate.

Nurse Practitioner or Physician Assistant — Skilled Nursing Facility

About the Role

We're looking for an experienced, autonomous Nurse Practitioner or Physician Assistant to join our clinical team full time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity — you'll manage your own panel and build long-term relationships with patients and their families.

Your Day-to-Day

You'll see roughly 16-20 patients a day — a mix of long-term chronic care and acute issues. That means annual wellness visits and H&Ps, managing chronic and co-morbid conditions, responding to acute changes, reviewing meds and orders, and coordinating with collaborating physicians, specialists, and facility staff to keep care seamless. You'll also stay in touch with families and join care conferences where it makes sense.
