Based at the West Orange clinic with flexible coverage, the role reports to the Associate Medical Director. Requires Florida license, NP/PA certification, and 1+ year in primary care or Medicare Advantage; includes base salary plus bonus and a robust benefits package.

Qualifications

• Master's Degree in Nursing or PA program with board certification.
• Active, unrestricted Florida license (NP or PA).
• Minimum one year experience in value-based care, primary care or Medicare Advantage.
• Preferred: Active DEA license and Medicaid/Medicare numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers.

Responsibilities

• You will provide comprehensive care to seniors following clinical standards.
• You will collaborate daily with your care team to ensure coordinated, high-quality care.
• You will manage referrals, hospital/SNF coordination, DME, and home health services.
• You will document care accurately and efficiently with support from quality-based coders.
• You will participate in on-call rotation and support growth plans at your center.

What We’re Looking For

• Required: Master's Degree in Nursing or completion of a PA program with board certification.
• Active, unrestricted NP or PA license in the state of Florida.
• Minimum one year Experience in value-based care, primary care or Medicare Advantage settings.
• Minimum one year experience as an APP Preferred: Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers Familiarity with HEDIS quality indicators and Medicare guidelines. #LIFM1 Use your skills to make an impact

• Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $123,800 - $170,300 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
