Job summary

Conviva Senior Primary Care in the San Antonio area (Boerne and Med Center clinics) is hiring a Nurse Practitioner or Physician Assistant to deliver value-based, high-quality care to seniors. Join a multidisciplinary team of clinicians, pharmacists, care coaches, and behavioral health specialists dedicated to personalized care.

You will work onsite at our Med Center and Boerne clinics, report to the Associate Medical Director, and contribute to coordinated, whole-person care for our patient

Qualifications

• Master's degree in Nursing or PA program with board certification.
• Active NP/PA license in state of practice.
• TB screening required.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with care team for coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care accurately with support from quality-based coders.
• Participate in on-call rotation and support growth plans at center.
• Provide value-based care.

As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.

In this role, you will work onsite at our Med Center and Boerne clinics located in the San Antonio area. You will report to our Associate Medical Director.

Your Role & Responsibilities

• You will provide comprehensive care to seniors following clinical standards.

Required

What We're Looking For

• You will have a Master's Degree in Nursing or completion of a PA program with board certification.
• You will have an active, unrestricted NP or PA license in the state of practice.
• If selected for this role, you will be required to be screened for TB.

Preferred

• You will have 2 or more years of experience in value-based care, primary care or Medicare Advantage settings.
• You will have an active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
