Job summary

Senior PsychCare is seeking a Nurse Practitioner or Physician Assistant to join a collaborative behavioral health team serving residents in long-term care facilities in Texas. You will conduct cognitive and emotional assessments, manage medications, and work with nursing staff and a dedicated psychiatry team.

Remote work is unlikely; travel within local facilities is required. This role emphasizes team-based care, flexibility, and compassionate, comprehensive patient management.

Qualifications

  • Licensed NP or PA in Texas.
  • Psychiatric/mental health experience required.
  • Geriatric populations experience preferred.
  • Spanish bilingual is a plus.
  • Valid driver’s license and ability to travel locally.
  • Adaptive, flexible, team-based care.

Responsibilities

  • Conduct cognitive and emotional assessments to set/manage treatment plans.
  • Manage psychotropic medication and gradual dose reductions.
  • Collaborate with nursing home staff and SPC’s behavioral health team.
  • Participate in treatment team meetings (in-person and virtual).
  • Deliver care both in-person and via telehealth when appropriate.
  • Maintain accurate, timely documentation in our EHR system.
