Job summary

Senior PsychCare is hiring a licensed NP or PA in Texas to provide geriatric mental health care in long-term care facilities, with options for in-person and telehealth visits.

You will conduct assessments, manage medications, and collaborate with a multidisciplinary team, supported by psychiatrists and staff to deliver compassionate, patient-centered care to residents. The role emphasizes continuity of care, documentation in the EHR, and flexible scheduling in a mission-driven environment.

Qualifications

Licensed PMHNP, FNP or PA in Texas.
Geriatric mental health experience required.
Spanish bilingual is a plus.
Valid driver’s license and ability to travel locally.
Adaptive and collaborative team player.

Responsibilities

Conduct cognitive and emotional assessments to set/manage treatment plans.
Manage psychotropic medication and gradual dose reductions.
Collaborate with nursing home staff and SPC’s behavioral health team.
Participate in treatment team meetings (in-person and virtual).
Deliver care both in-person and via telehealth when appropriate.
Maintain accurate, timely documentation in our EHR system.
