Job summary

Bluestone Physician Services invites applications for an on-site Nurse Practitioner or Physician Assistant role serving Assisted Living and Memory Care communities around Eagan, MN. You will provide team-based primary care with clinical support from LPNs, RNs, and social workers.

The position offers a full-time schedule (Mon-Thu, with optional Friday either clinical or admin), a patient panel of ~200, and schedule design autonomy.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program.
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or ability to obtain licensure.
• Ability to obtain and/or maintain DEA certification as required by state.

Responsibilities

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities.
• Full-time providers care for patients Monday-Thursday; option for Fridays for clinical or administrative day.
• Manage a panel of 200 patients (12-16 per day, ~14 avg).
• Design and manage your schedule; develop and implement care plans.
• Clinical support from LPNs, RNs and Social Workers; access to labs and x-rays.
• Collaborate with home care, hospice and community resources.
