Job summary

HarmonyCares is seeking a Nurse Practitioner to provide in-home primary care for high-acuity geriatric patients in NW Chicago. The role emphasizes patient-centered care with a Monday–Friday schedule, no weekends or evenings, and managing chronic conditions within a geriatric panel.

You will be supported by an integrated care team and receive a comprehensive benefits package, CME/licensing stipend, malpractice tail coverage, and UpToDate access. Experience in home health or geriatrics preferred.

Qualifications

• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Must maintain a valid driver’s license and good driving record

• Provide home-based primary care to high-acuity geriatric patients.

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
