Job summary

HarmonyCares seeks a Nurse Practitioner to provide in-home primary care to high-acuity patients in dedicated markets. You will focus on patient-centered care, taking the time needed to address complex needs and prevent hospitalizations.

Responsibilities include collaboration with an interdisciplinary team, completing assessments, and coordinating ongoing care plans. Must hold NP licensure and DEA eligibility, with geriatric experience preferred.

Qualifications

• Licensed NP in the state where HarmonyCares operates.
• Federal DEA license or eligibility is required.
• CPR/BLS certification must be current.
• Board certification is preferred/required by role.
• Valid driver's license and clean driving record are required.
• Geriatric training or experience is preferred.

Responsibilities

• Provide in-home primary care to high-acuity patients in assigned markets.
• Deliver patient-centered care with time for assessment and planning.
• Collaborate with an interdisciplinary team to coordinate care.

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
