Job summary

HarmonyCares in Denton, TX seeks a Geriatric Advanced Practice Provider for home-based primary care in a value-based model. The salaried role covers Denton, Aubrey, Lewisville and Carrollton with 30–45 minute visits, seeing about 6–9 patients per day, Monday–Friday, no weekends or evenings.

Qualifications

• Licensed NP in the state of practice
• Federal DEA license/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS certification
• Board Certification

Responsibilities

• Provide home-based primary care to geriatrics in patients' homes
• Deliver patient-centered care with focus on quality over volume
• Coordinate care with an interdisciplinary team
• Manage visits in Denton, Aubrey, Lewisville and Carrollton

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
