Primary Vascular Care is a mobile vascular group delivering clinic‑like care directly to nursing homes across the New York City area. We provide comprehensive vascular assessments and advanced care in residents’ familiar environments.

We are seeking a Nurse Practitioner or Physician Assistant to perform on‑site clinical assessments, determine treatment plans, and support facility teams. Travel and collaboration with the Lead NP are essential to ensure high‑quality, patient‑centered care.

Qualifications

• Must hold a Bachelor’s Degree.
• Graduate from an accredited NP or PA program.
• Current NP license required.
• BLS through American Heart Association required.
• ACLS preferred.
• Current Infection Control Certification required.
• Board certification or eligibility.
• Vaccines: PPD/TB, Hep B, MMR, Varicella required.
• COVID and Flu vaccines preferred.
• 1 year NP experience preferred.
• Reliable transportation in personal vehicle.

Responsibilities

• Perform on-site screening assessments to determine treatment plans.
• Conduct history and physicals for perioperative patients.
• Order and review diagnostic and therapeutic tests as age-specific needs.
• Assist with care in accordance with facility, state, and federal regulations.
• Monitor effectiveness of interventions.
• Maintain relationships with facility contacts.
• Educate families and clients about PV Care services.
• Assist with physician relations as assigned.
• Complete weekly, monthly, and other reports timely.

Primary Vascular Care is looking for a Nurse Practitioner (NP) or Physician Assistant (PA) to perform on-site clinical assessments to determine appropriate patient treatment plans of residents of client facilities. This position reports to the Lead Nurse Practitioner.

The NP/PA will work in a mobile practice, primarily serving a long-term care setting.

Must be able to travel throughout the New York City area.

ESSENTIAL FUNCTIONS:

• Performs on-site screening clinical assessments to determine appropriate patient treatment plans of residents of client facilities by the completion of a comprehensive assessment form
• Functions independently to perform appropriate history and physical for complex acute, critical, and chronically ill perioperative patients
• Orders and reviews diagnostic and therapeutic tests relative to patient’s age-specific needs
• Assists with the provision of care in accordance with facility, state, and federal regulations
• Monitors the effectiveness of interventions
• Maintains relationships with all appropriate contacts in facilities
• Educates family members, clients, and prospective accounts regarding services and programs offered by Primary Vascular Care
• Assists with the development and maintenance of physician relations, as assigned
• Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner

QUALIFICATIONS:

• Bachelor’s Degree required
• Degree from an accredited Nurse Practitioner or Physician Assistant Program required
• Current NP License required
• BLS - Basic Life Support through American Heart Association required
• ACLS - Advanced Cardiac Life Support through American Heart Association preferred
• Current Infection Control Certification required
• Board Certified/Board Eligible required
• Physical, Vaccines and Titers: PPD/TB, Hep B, MMR, Varicella required
• COVID and Flu Vaccine preferred
• 1 year of NP experience preferred
• Reliable transportation (personal vehicle) required
