Job summary

Primary Vascular Care is a specialized vascular group serving the long-term care space, providing clinic-like vascular care in nursing homes. The NP/PA will perform on-site clinical assessments to determine appropriate patient treatment plans for residents in client facilities, reporting to the Lead NP.

This mobile practice primarily serves long-term care settings across the New York City area. Requirements include a Bachelor's degree, NP or PA program accreditation, current NP license, BLS

Qualifications

• Bachelor’s Degree required.
• Degree from an accredited Nurse Practitioner or Physician Assistant Program required.
• Current NP License required.
• BLS – Basic Life Support required.
• ACLS – Advanced Cardiac Life Support preferred.
• Current Infection Control Certification required.
• Board Certified/Board Eligible required.
• Vaccines and Titers: PPD/TB, Hep B, MMR, Varicella required.
• COVID and Flu Vaccine preferred.
• 1 year of NP experience preferred.
• Reliable transportation (personal vehicle) required.

Responsibilities

• Performs on-site screening clinical assessments to determine appropriate patient treatment plans of residents of client facilities by completion of a comprehensive assessment form.
• Functions independently to perform history and physical for complex perioperative patients.
• Orders and reviews diagnostic and therapeutic tests relative to patient’s age-specific needs.
• Assists with the provision of care in accordance with facility, state, and federal regulations.
• Monitors the effectiveness of interventions.
• Maintains relationships with all appropriate contacts in facilities.
• Educates family members, clients, and prospective accounts regarding services and programs offered by Primary Vascular Care.
• Assists with the development and maintenance of physician relations, as assigned.
• Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner.

Primary Vascular Care is looking for a Nurse Practitioner (NP) or Physician Assistant (PA) to perform on‑site clinical assessments to determine appropriate patient treatment plans of residents of client facilities. This position reports to the Lead Nurse Practitioner.

The NP/PA will work in a mobile practice, primarily serving a long‑term care setting.

Must be able to travel throughout the New York City area.

ESSENTIAL FUNCTIONS:

• Performs on‑site screening clinical assessments to determine appropriate patient treatment plans of residents of client facilities by completion of a comprehensive assessment form.

• Orders and reviews diagnostic and therapeutic tests relative to patient’s age‑specific needs.
• Assists with the provision of care in accordance with facility, state, and federal regulations.

• Maintains relationships with all appropriate contacts in facilities.
