Integra Vascular in New York seeks a Nurse Practitioner or Physician Assistant to deliver on-site vascular care at client facilities. You will perform screening assessments, develop treatment plans, and work closely with physicians to optimize patient outcomes.

This role requires current NP/PA licensure, ACLS, infection control certification, and strong EMR/PACS experience. A valid driving record, a personal vehicle, and the ability to travel locally are essential.

Qualifications

• Current Nurse Practitioner/ Physician Assistant License.
• Degree from an accredited Nurse Practitioner/ Physician Assistant Program.
• ACLS.
• Current infection control certification.
• Strong clinical collaboration with physicians.
• Proficient computer skills.
• PACS and EMR experience preferred.
• Committed to professional growth and learning.
• Clean driving record.
• Personally owned vehicle required.
• Local travel required.
• Not listed on the OIG exclusion list.

Responsibilities

• Performs on-site screening clinical assessments to determine appropriate patient treatment plans of residents at client facilities by the completion of a comprehensive assessment.
• Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill perioperative patients.
• Orders and interprets diagnostic and therapeutic tests relative to patient's age-specific needs.
• Assists with the provision of care in accordance with facility, state, and federal regulations.
• Monitors the effectiveness of interventions.
• Maintains relationships with all appropriate contacts in facilities.
• Educates family members, clients, and prospective accounts regarding services and programs.
• Assists with the development and maintenance of physician relations, as assigned.
• Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner.
• Other Duties as assigned by Medical Director
