Job summary

Primary Vascular Care is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to perform on-site assessments for residents of client facilities in the New York City area. You will work under the Lead NP to determine appropriate treatment plans and ensure high-quality care across facilities.

The role requires a valid NP license or PA certification, BLS, and infection control credentials, with ACLS preferred. Travel throughout the NYC area is essential, and prior NP experience is favored.

Qualifications

• Bachelor’s Degree required.
• Degree from an accredited Nurse Practitioner or Physician Assistant Program required.
• Current NP License required.
• BLS Certification required.
• ACLS Certification preferred.
• Current Infection Control Certification required.
• Board Certified or Board Eligible.
• Vaccinations: PPD/TB, Hep B, MMR, Varicella required; COVID and Flu vaccines preferred.
• 1 year NP experience preferred.
• Reliable transportation required.

Responsibilities

• Performs on-site screening assessments to determine patient treatment plans for residents.
• Conducts history and physical exams for complex perioperative patients.
• Orders and reviews diagnostic tests as age-specific needs.
• Assists with care in accordance with facility, state, and federal regulations.
• Monitors the effectiveness of interventions.
• Educates family members, clients, and facilities about services offered.
• Assists with development and maintenance of physician relations.
• Completes reports weekly, monthly, and as assigned.

Primary Vascular Care is looking for a Nurse Practitioner (NP) or Physician Assistant (PA) to perform on-site clinical assessments to determine appropriate patient treatment plans of residents of client facilities. This position reports to the Lead Nurse Practitioner.

Must be able to travel throughout the New York City area.

ESSENTIAL FUNCTIONS

• Performs on-site screening clinical assessments to determine appropriate patient treatment plans of residents of client facilities by the completion of a comprehensive assessment form
• Functions independently to perform appropriate history and physical for complex acute, critical, and chronically ill perioperative patients
• Orders and reviews diagnostic and therapeutic tests relative to patient’s age-specific needs
• Assists with the provision of care in accordance with facility, state, and federal regulations
• Monitors the effectiveness of interventions
• Maintains relationships with all appropriate contacts in facilities
• Educates family members, clients, and prospective accounts regarding services and programs offered by Primary Vascular Care
• Assists with the development and maintenance of physician relations, as assigned
• Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner

QUALIFICATIONS

• Bachelor’s Degree required
• Degree from an accredited Nurse Practitioner or Physician Assistant Program required
• Current NP License required
• BLS - Basic Life Support through American Heart Association required
• ACLS - Advanced Cardiac Life Support through American Heart Association preferred
• Current Infection Control Certification required
• Board Certified/Board Eligible required
• Physical, Vaccines and Titers: PPD/TB, Hep B, MMR, Varicella required
• COVID and Flu Vaccine preferred
• 1 year of NP experience preferred
• Reliable transportation (personal vehicle) required
