Job summary

Enhabit Home Health & Hospice is seeking a licensed Nurse Practitioner to join our interdisciplinary hospice team. You will provide direct medical care to patients, determine hospice eligibility, and manage pain and symptoms as part of the care plan.

Qualifications

Qualifications

Education and experience, essential

• Must be a graduate of an approved school, and licensed as a nurse practitioner (NP) in the state in which they currently practice.
• Must have demonstrated knowledge of regulatory requirements, including Medicare conditions of participation.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

• At least five years nursing experience with a minimum of one year in hospice preferred.
• At least three years of clinical registered NP experience preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification
