Job summary

Enhabit Home Health & Hospice in Corpus Christi, Texas seeks a Nurse Practitioner to join our hospice interdisciplinary team delivering compassionate medical care to patients at home and in community settings. You will assess and manage pain and symptoms, determine hospice eligibility, collaborate with physicians and care coordinators, and participate in care planning.

Responsibilities

Provide medical care to hospice patients. Determine hospice eligibility and pain and symptom management. Participate in the care of the hospice patient as a member of the interdisciplinary team

Qualifications

Education and experience, essential

• Must be a graduate of an approved school, and licensed as a nurse practitioner (NP) in the state in which they currently practice.
• Must have demonstrated knowledge of regulatory requirements, including Medicare conditions of participation.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education And Experience, Preferred

• At least five years nursing experience with a minimum of one year in hospice preferred.
• At least three years of clinical registered NP experience preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification
