Job summary

Enhabit Home Health & Hospice is seeking a Nurse Practitioner to join our hospice care team in Texas. You will provide expert medical care to patients at home and work within an interdisciplinary team to coordinate care and improve comfort and outcomes.

Responsibilities include determining hospice eligibility, managing pain and symptoms, and collaborating with physicians, nurses, and social workers to support patients and families in their home setting.

Qualifications

• Must be a licensed NP in the state of practice.
• Knowledge of Medicare conditions of participation.
• Proficient with mobile devices and basic technology.

Responsibilities

Provide medical care to hospice patients. Determine hospice eligibility and pain and symptom management. Participate in the care of the hospice patient as a member of the interdisciplinary team.

Qualifications

Education and experience, essential

• Must be a graduate of an approved school, and licensed as a nurse practitioner (NP) in the state in which they currently practice.
• Must have demonstrated knowledge of regulatory requirements, including Medicare conditions of participation.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education and Experience, Preferred

• At least five years nursing experience with a minimum of one year in hospice preferred.
• At least three years of clinical registered NP experience preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification
