Job summary

Enhabit Home Health & Hospice in Mesa, AZ is seeking a Nurse Practitioner to join our hospice care team. You will provide medical care to hospice patients, assess pain and symptoms, and participate in interdisciplinary team planning to ensure compassionate, high-quality care at home.

The ideal candidate is licensed as an NP in Arizona, graduates from an approved school, and has knowledge of Medicare conditions of participation.

Qualifications

• Must be a graduate of an approved school and licensed as an NP in the state of practice.
• Must have knowledge of Medicare conditions of participation.
• Must have basic technology skills, including operation of a mobile device.

Responsibilities

• Provide medical care to hospice patients.
• Determine hospice eligibility and pain and symptom management.
• Participate in the care of the hospice patient as a member of the interdisciplinary team.

Education and experience, essential

• Must be a graduate of an approved school, and licensed as a nurse practitioner (NP) in the state in which they currently practice.
• Must have demonstrated knowledge of regulatory requirements, including Medicare conditions of participation.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education And Experience, Preferred

• At least five years nursing experience with a minimum of one year in hospice preferred.
• At least three years of clinical registered NP experience preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification
