Job summary

Windom Area Health in Fairmont, MN seeks a Nurse Practitioner (NP) to deliver high-quality Primary Care and Urgent Care for patients of all ages. This role provides timely, compassionate care close to home across southwest Minnesota.

The NP will evaluate and treat acute and chronic conditions, conduct exams, order tests, and coordinate with the care team. Minnesota-licensed CNP with DEA credentials is required; BLS is required, ACLS/PALS preferred.

Qualifications

• Graduate of an accredited Certified Nurse Practitioner (CNP) program.
• Minimum of one year of Primary Care, Family Practice, Internal Medicine, Emergency Medicine, or Urgent Care experience preferred.
• Certified Nurse Practitioner (CNP), licensed in Minnesota. Ability to obtain and maintain DEA certification for Minnesota. Meet credentialing/privileging thresholds. BLS Certified. ACLS and/or PALS certification preferred.

Responsibilities

• Provide comprehensive Primary Care and Urgent Care services for pediatric and adult patients.
• Evaluate, diagnose, and treat acute illnesses, injuries, and chronic medical conditions.
• Deliver preventive care, wellness screenings, and health education.
• Conduct comprehensive and focused patient assessments, histories, and physical examinations.
• Order and interpret diagnostic testing and develop individualized treatment plans.
• Prescribe medications and therapies per guidelines and scope of practice.
• Perform office-based procedures and interventions.
• Coordinate care with physicians, nursing staff, specialists, and community resources.
• Advocate for patients and connect them with services that improve long-term health outcomes.
• Foster continuity of care through meaningful relationships and ongoing patient engagement.

Requirements

Required credentials:

Certified Nurse Practitioner (CNP), licensed in the state of Minnesota. Ability to obtain and maintain DEA certification for the State of Minnesota. Meet threshold criteria/qualifications for Credentialing and Privileging. BLS Certified. ACLS and/or PALS certification preferred.

This position would work 3 ten-hour shifts per week, plus every other weekend (4 hours each day).
