Job summary

Twin Cities Physicians seeks a Geriatric Wound Care Nurse Practitioner (AGNP/CNP) to provide on-site and mobile rounding across assisted living communities and skilled nursing facilities in the Twin Cities Metro.

You will assess wounds, plan treatment, and coordinate care with physicians and care teams, while advancing geriatrics and post-acute medicine. This full-time role features a dynamic, community-based practice with broad impact.

Qualifications

• Active Minnesota CNP license.
• Experience in wound care, geriatrics, primary or post-acute care desirable.
• Valid driver's license and ability to travel throughout Twin Cities Metro.
• Strong clinical judgment for medically complex patients.

Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

What You'll Be Doing

• Provide advanced wound care assessments, treatment planning, and ongoing management.

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license.

• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
• Strong clinical judgment and confidence managing medically complex patient populations.
