Job summary

Purple Cow Recruiting is seeking a board-certified Nurse Practitioner for a Primary Care outpatient role in Montgomery, Alabama. You will care for seniors with multiple chronic conditions in a value-based setting, with a dedicated care team and strong leadership support.

Shifts are Monday to Friday, with a base salary of $130,000 plus up to 10% bonus, and a total compensation potential of about $143,000+ per year along with comprehensive benefits.

Qualifications

• Must be a board-certified Nurse Practitioner with unrestricted license.
• 2+ years of Primary Care Outpatient experience in geriatrics.
• Current, active DEA registration.
• Proficient in office-based procedures: suturing, joint injections, skin biopsies, basic wound care.

Responsibilities

• Provide primary outpatient care for seniors with complex conditions (3–5 diseases).
• Manage patient panels and coordinate with care team to deliver value-based care.
• Maintain high patient satisfaction and adherence to HEDIS & quality metrics.
• Collaborate with leadership and support staff to optimize clinic operations.

COMPENSATION: TOTAL Potential $143,000+ Full Benefits

The center provides Uber/Lyft to those who don’t drive. Also, they offer exercise, art and other activities for their seniors to promote better health at no cost to the patient.

Role

• Compensation: Base $130,000 + up to 10% of base salary in bonus’
• Primary Care – Outpatient
• Start date: ASAP
• Value Based Care Model
• Patient population: Seniors with 3-5 complex diseases (Hearth disease, diabetes, hypertension etc.)
• Average patients per day: 10-12
• Panel size: Max of 650
• Shifts: Monday-Friday 8a-5p
• Call Schedule: Based on number of providers in the market (if 32 physicians in market–call 1:32wks)
• Nurse Practitioners have their own care team with MA/scheduler/ referral specialist and full support from leadership

Requirements

• Nurse Practitioner, board certified
• 2+ years of Primary Care Outpatient Experience
