Care Partners in California is seeking an in-clinic Nurse Practitioner with geriatric experience to evaluate patients, formulate treatment plans, and coordinate care across nursing homes and assisted living facilities.

This full-time role offers a Monday–Friday 8am–5pm schedule, no in-home visits, 5–8 patients per day, and a competitive salary of $150,000-$180,000 with open negotiation based on experience, plus comprehensive benefits including 100% covered medical, dental and vision, 401k match,

Qualifications

• Active Nurse Practitioner license in California.
• Geriatric care experience preferred.
• Current active DEA license.

• Experience in elderly care, 1 year experience as a nurse practitioner in a geriatric setting/ working with the frail or elderly is required.
• Current active DEA license

Essential Functions:

• Conducts physical exams, diagnostic tests, blood draws, EKGs, dressing changes, and other nursing interventions.
• Assesses patients' conditions, formulates treatment plans including prescriptions, and provides ongoing follow-up.
• Educates staff, participants, and caregivers on healthcare issues and treatments.
• Manages acute and long-term medical needs in collaboration with other providers, covering care in various settings including nursing homes and assisted living facilities.
• Provides emotional and spiritual support, manages medical emergencies, and ensures timely, compliant documentation in medical records.
