Job summary

Paylocity in Kansas City, MO is seeking a Nurse Practitioner for a primary care outpatient role serving seniors with complex diseases in a value-based care model. The team includes MA/scheduler/referral support and is aligned with leadership to improve patient outcomes.

The compensation includes a base of $130,000 plus up to 10% of base salary in bonuses, with total potential around $143,000+. Start date ASAP. Shifts Monday-Friday 8a-5p; travel not required.

Qualifications

• Nurse Practitioner must be board certified.
• 2+ years in primary care outpatient setting.
• Experience with geriatric patients and chronic disease management.

Responsibilities

• Provide primary care to seniors with complex diseases in outpatient setting.
• Collaborate with care team including MA and referral specialists.
• Adhere to value-based care model and quality metrics.
• Perform basic procedures: suturing, joint injections, skin biopsies, wound care.

Role:

• Compensation: Base $130,000 + up to 10% of base salary in bonus’
• Primary Care – Outpatient
• Start date: ASAP
• Value Based Care Model
• Patient population: Seniors with 3-5 complex diseases (Hearth disease, diabetes, hypertension etc.)
• Average patients per day: 10-12
• Panel size: Max of 650
• Shifts: Monday-Friday 8a-5p
• Call Schedule: Based on number of providers in the market (if 32 physicians in market–call 1:32wks)
• Nurse Practitioners have their own care team with MA/scheduler/ referral specialist and full support from leadership

Requirements:

• Nurse Practitioner, board certified
• 2+ years of Primary Care Outpatient Experience
• Geriatrics experience
• Current, unrestricted, active license to practice medicine
• Current DEA registration
• Proficient PC skills in office procedures: suturing, joint injections, skin biopsies, basic wound care - REQUIRED
