Job summary

St. Joseph Hospice in the Houston area is seeking an experienced PRN Nurse Practitioner to join our hospice healthcare team.

This role focuses on delivering compassionate, home-based primary and palliative care to patients who are home-bound or facing travel challenges. You will perform physical exams, diagnose and manage chronic conditions, interpret tests, and prescribe medications with an emphasis on comfort and quality of life.

Qualifications

• Current TX NP license is required.
• Two+ years of Nurse Practitioner experience.
• Hospice or palliative care experience preferred.
• Excellent communication skills.
• Problem-solving abilities and adaptability.
• Experience with EMR systems preferred.

Responsibilities

• Provide primary and palliative care for home-bound patients.
• Perform physical exams and diagnose chronic/acute conditions.
• Interpret lab results and adjust treatments.
• Prescribe medications and therapies with prevention focus.

Essential Daily Duties Include

• Provide primary medical care and palliative care for chronic and acute illnesses to patients who are home-bound or have difficulty traveling to a physician’s office
• Provide physical exams, diagnose and treat acute and chronic diseases
• Interpret laboratory and test results
• Prescribe and manage medications and other therapies all with an emphasis on health maintenance and disease prevention

Qualifications

• Current TX license required
• Two or more years as a Nurse Practitioner
• Excellent communication skills
• Problem-solving skills and ability to deal with a variety of situations
• Hospice or Palliative care experience preferred
• Experience EMR preferred
