Job summary

St. Croix Hospice seeks a compassionate Nurse Practitioner to support patients and families during advanced illness, providing skilled medical care with calm guidance.

The NP ensures continuity of care through medication oversight, treatments, and recertifications. You will collaborate with interdisciplinary teams, educate families on end-of-life options, and help honor patients’ values and goals while maintaining HIPAA standards and thorough documentation.

Qualifications

• Current NP license in the state(s) of practice.
• Degree from an accredited NP program.
• Minimum two years of RN experience; hospice or palliative care experience preferred.
• Proficient in electronic health records and HIPAA compliant.

Responsibilities

• Provide skilled medical care for terminally ill patients, including assessments and care plans.
• Prescribe medications and complete hospice recertifications.
• Collaborate with interdisciplinary teams to manage symptoms and ensure continuity of care.
• Educate and support patients, families, and caregivers on end-of-life care.

Qualifications

Nurse Practitioner Requirements & Key Responsibilities:

• Must have current Nurse Practitioner (NP) license in the state(s) of practice.
• Degree from an accredited NP program required.
• Minimum two years of RN experience; preferred hospice or palliative care experience.
• Provide comprehensive medical care for terminally ill patients, including assessments, diagnostic interpretation, and individualized care plans.
• Prescribe medications/treatments, complete hospice recertification face‑to‑face assessments, and ensure compliance with state and federal regulations.
• Collaborate with interdisciplinary teams and physicians to manage symptoms and maintain continuity of care.
• Educate and support patients, families, and caregivers on end‑of‑life care, including medical, emotional, and spiritual needs.
• Maintain accurate documentation, adhere to HIPAA standards, and participate in quality improvement initiatives.
• Demonstrate strong clinical judgment, communication, and problem‑solving skills; proficient in electronic health records.
• Valid driver’s license, reliable transportation, automobile insurance, and ability to pass background and drug screening required.
