Job summary

St. Joseph Hospice is hiring a Full-Time Hospice Registered Nurse (RN) in Houston, TX. You will assess patients, implement the Hospice Plan of Care, and educate families in care planning and medications.

The role requires a Texas RN license, CPR, and EMR knowledge, with at least two years of RN experience, preferably in hospice or related settings. Join a team that emphasizes patient-centered care and collaboration with physicians, families, and the hospice staff.

Qualifications

  • RN license in Texas required.
  • Minimum of two years full-time RN experience in hospice or related settings.
  • Current CPR certification required.
  • Valid driver’s license and auto insurance.
  • Knowledge of EMR required.
  • Hospice experience preferred.

Responsibilities

  • Performs initial and ongoing patient assessment.
  • Assist in developing and implementing the Hospice Plan of Care.
  • Educates patient and family on Plan of Care and medications.
  • Act as liaison between physician, patient/family and hospice team.
  • Maintain accurate medical records.
  • Make routine supervisory visits.

Job Duties
  • Performs the initial and on-going assessment of the patient
  • Assist in developing and implementing the Hospice Plan of Care
  • Educate patient and family on Plan of Care, including medication administration, dressing changes, oxygen administration, Hospice philosophy and care of the terminally ill patient
  • Act as primary liaison between physician, patient/family and hospice team
  • Maintain an accurate and updated medical record
  • Makes routine supervisory visits
  • Participates in agency on-call schedule
  • Abide by company policies and procedures, compliance guidelines, applicable laws, and regulations promptly reporting any suspected violation of compliance standards
Qualifications
  • Registered Nurse (RN) license required in State of Texas
  • Minimum of two (2) years full-time experience as an RN, either in oncology, an acute care setting with direct patient care, a long-term care facility, hospice or home health care. Two (2) of experience in hospice care as an LPN may be substituted for the required 2 years of experience as an RN
  • Current CPR certification required
  • Current automobile insurance and valid driver’s license
  • Knowledge of EMR is required
  • Previous Hospice experience preferred
  • Compassionate RN with great time management skills
