Job summary

St. Joseph Hospice is seeking a qualified Nurse Practitioner to join our hospice care team in the Houston, TX area on a PRN basis. You will provide primary and palliative care to home-bound patients, perform exams, interpret tests, and manage medications with an emphasis on health maintenance.

TX license required with 2+ years of NP experience. Experience in hospice or palliative care is preferred, and EMR experience is a plus.

Qualifications

• Current TX license required.
• Two or more years as a Nurse Practitioner.
• Strong problem-solving skills and ability to handle varied situations.
• Hospice or Palliative care experience preferred; EMR experience a bonus.

Responsibilities

• Provide primary medical and palliative care to home-bound patients.
• Perform physical exams, diagnose and treat acute and chronic illnesses.
• Interpret laboratory and test results.
• Prescribe and manage medications and therapies with emphasis on prevention.

Essential daily duties include:

• Provide primary medical care and palliative care for chronic and acute illnesses to patients who are home-bound or have difficulty traveling to a physician’s office
• Provide physical exams, diagnose and treat acute and chronic diseases
• Interpret laboratory and test results
• Prescribe and manage medications and other therapies all with an emphasis on health maintenance and disease prevention

Qualifications:

• Experience EMR preferred
