Job summary

The Carpenter Health Network is seeking an experienced PRN Nurse Practitioner to join our hospice care team in the Houston, TX area. You will provide primary medical and palliative care to home-bound patients, and collaborate with a multidisciplinary team to coordinate comfort-focused treatment.

Applicants must hold a current TX license and have at least two years of NP experience, preferably in hospice or palliative care.

Qualifications

• Current TX license required.
• Two or more years as a Nurse Practitioner.
• Problem-solving skills and ability to deal with a variety of situations.
• Hospice or Palliative care experience preferred.
• EMR experience preferred.

Responsibilities

• Provide primary medical and palliative care for home-bound patients.
• Perform physical exams, diagnose and treat acute and chronic diseases.
• Interpret laboratory and test results.
• Prescribe and manage medications and other therapies with focus on prevention.

Essential daily duties include:

• Provide primary medical care and palliative care for chronic and acute illnesses to patients who are home-bound or have difficulty traveling to a physician’s office
• Provide physical exams, diagnose and treat acute and chronic diseases
• Interpret laboratory and test results
• Prescribe and manage medications and other therapies all with an emphasis on health maintenance and disease prevention

Qualifications:

• Current TX license required

• Experience EMR preferred
