Job summary

St. Joseph Hospice in the Houston, TX area is seeking an experienced PRN Nurse Practitioner to join our hospice care team. The role focuses on delivering high-quality primary and palliative care to home-bound patients.

Qualifications include TX license and 2+ years as an NP, with strong communication and problem-solving skills; hospice or palliative care experience and EMR familiarity are preferred. This is a part-time opportunity with benefits.

Qualifications

• Hospice or Palliative care experience preferred.
• EMR experience preferred.

Responsibilities

• Provide primary medical and palliative care for home-bound patients.
• Perform physical exams, diagnose and treat acute/chronic diseases.
• Interpret laboratory and test results.
• Prescribe and manage medications and other therapies with emphasis on health maintenance and disease prevention.

Essential daily duties include:

• Provide primary medical care and palliative care for chronic and acute illnesses to patients who are home-bound or have difficulty traveling to a physician’s office
• Provide physical exams, diagnose and treat acute and chronic diseases
• Interpret laboratory and test results
• Prescribe and manage medications and other therapies all with an emphasis on health maintenance and disease prevention

Qualifications:
