Job summary

St. Joseph Hospice in Houston, TX is hiring a Full-Time Hospice Registered Nurse (RN) to join our care team. The role provides direct patient care in a hospice setting with a $5,000 sign-on bonus and eligibility for ESOP stock ownership.

The RN will assess patients, implement the Plan of Care, educate families on care and medications, coordinate with physicians, and maintain accurate records while participating in on-call coverage. Experience in hospice preferred; CPR and TX RN license required.

Qualifications

• RN license required in State of Texas.
• Minimum of two years full-time RN experience in oncology, acute care with direct patient care, long-term care, hospice or home health care.
• Two years of hospice experience preferred.
• Current CPR certification.
• Valid driver’s license and automobile insurance.
• Knowledge of EMR is required.

Responsibilities

• Performs the initial and on-going assessment of the patient.
• Assist in developing and implementing the Hospice Plan of Care.
• Educate patient and family on Plan of Care, including medication administration, dressing changes, oxygen administration, Hospice philosophy and care of the terminally ill patient.
• Act as primary liaison between physician, patient/family and hospice team.
• Maintain an accurate and updated medical record.
• Makes routine supervisory visits.
• Participates in agency on-call schedule.
• Abide by company policies and procedures, compliance guidelines, applicable laws, and regulations promptly reporting any suspected violation of compliance standards.

• Minimum of two (2) years full-time experience as an RN, either in oncology, an acute care setting with direct patient care, a long-term care facility, hospice or home health care. Two (2) of experience in hospice care as an LPN may be substituted for the required 2 years of experience as an RN
• Current CPR certification required
• Current automobile insurance and valid driver’s license
• Knowledge of EMR is required
• Previous Hospice experience preferred
• Compassionate RN with great time management skills
