Job summary

St. Joseph Hospice in LOCATION is hiring a Registered Nurse (RN) to join a team delivering exceptional patient care in a home health and hospice setting. The role requires an RN license in the state, at least two years of full-time RN experience, CPR certification, and EMR familiarity.

Ideal candidates will have hospice or home health experience, a valid driver's license with automobile insurance, and strong time management. On-site position with competitive pay and benefits.

Qualifications

• Registered Nurse (RN) license required in the State of LOCATION.
• Minimum of two years full-time experience as an RN in oncology, acute care, long-term care, hospice or home health care.
• Current CPR certification required.
• Current automobile insurance and valid driver’s license.
• Knowledge of EMR is required.
• Previous Home Health or Hospice experience preferred.
• Compassionate RN with great time management skills.

Responsibilities

• Performs the initial and on-going assessment of the patient.
• Assist in developing and implementing the Hospice Plan of Care.
• Educate patient and family on Plan of Care, including medication administration and care of the terminally ill patient.
• Act as primary liaison between physician, patient/family and hospice team.
• Maintain an accurate and updated medical record.
• Makes routine supervisory visits.
• Participates in agency on-call schedule.
• Adhere to policies, compliance guidelines and laws; report any suspected violations.

Job Duties:
