Zusammenfassung

SAC Health is seeking a Nurse Practitioner to join the Message Center, integrating advanced nursing processes to deliver high-quality outpatient care. You will coordinate care, develop treatment plans, and collaborate with interdisciplinary teams to optimize outcomes and patient satisfaction.

Requirements include a Master's in Nursing, active California RN and NP licensure, Epic certification, and 1–2 years of related experience.

Qualifikationen

Master's degree required; NP curriculum completed.
California RN license and NP certification required. DEA license recommended.
Active California NP license with certification required. EPIC certification required for module.
Minimum one year NP specialty experience; two years RN experience in acute care or sub-specialty clinic.

Aufgaben

Manage call queues and triage within NP scope of practice.
Coordinate patient referrals and education for patients and families.
Serve as clinical resource to staff, physicians, patients, and other departments.
Conduct comprehensive nursing assessments and perform diagnostic/therapeutic procedures.
Collaborate with multidisciplinary teams to coordinate complex care plans for chronic conditions.
