Job summary

CareOne in East Longmeadow, MA is seeking an Advanced Practice Nurse to join Bridge Care. The role involves conducting visits, documenting findings, and coordinating care with an interdisciplinary team to deliver high-quality resident care.

The ideal candidate holds an MSN or DNP and an active Nurse Practitioner license in MA, with at least 1 year in Family, General or geriatric practice, plus a valid driver's license and willingness to travel.

Qualifications

• Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
• Must have a Nurse Practitioner's license in MA.
• Minimum 1 year of experience in Family Practice, General Practice, or geriatric care.

Responsibilities

• Conduct visits to patients in accordance with local, state and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team.
• Facilitate access to appropriate support models of care (rehabilitation, ongoing aggressive treatment).
• Consult with other members of the interdisciplinary team to contribute to the patient's plan of care.
• Prepare and maintain accurate patient records, charts and documents.
• Assist in development of clinical practice guidelines/standards to support quality care.
• Ensure collaboration with patients, caregivers, medical directors and IDT members.

Salary Range $125,000 to $135,000 a year

Job Description

Advanced Practice Nurse will be responsible for, but are not limited to:

• Conduct visits to patients in accordance with local, state and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team.
• Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment).
• Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
• Prepare and maintain accurate patient records, charts and documents.
• Assist in development of clinical practice guidelines/standards in support of quality care as needed.
• Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Education

Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.

Professional Experience

• Must have a Nurse Practitioner's license in the state of operations
• Must have 1+ years' experience working within Family Practice, General Practice, Geriatric care or related field,
• Must have a valid driver's license and a reliable source of transportation
• Must be willing to travel
• Experience in a nursing home, hospital, assisted living or acute rehab space is a plus but not required.
