Job summary

CareOne in Massachusetts is seeking a Psychiatric-Mental Health Nurse Practitioner to provide high-quality care in our long-term care setting. The PMHNP will conduct visits and coordinate with the interdisciplinary team.

Requirements: MSN or DNP, MA NP license, PMHNP certification, 1+ year in family, geriatric or general practice; local travel is required. Salary ranges from $125,000 to $140,000 annually with comprehensive benefits.

Qualifications

• Must hold a valid Massachusetts NP license.
• PMHNP certification required.
• Minimum 1 year of experience in family, geriatric or general practice.

Responsibilities

• Conduct visits to patients in accordance with regulations.
• Communicate visit findings to the interdisciplinary team.
• Facilitate access to appropriate care models and treatment.
• Consult with the team to contribute to patient plans.
• Prepare and maintain patient records and charts.
• Contribute to clinical practice guidelines as needed.
• Ensure collaboration with patients, caregivers, medical directors and IDT.

Salary Range

$125,000 to $140,000 a year

Advanced Practice Nurse

The Advanced Practice Nurse will be responsible for, but are not limited to:

• Conduct visits to patients in accordance with local, state and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team.
• Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment).
• Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
• Prepare and maintain accurate patient records, charts and documents.
• Assist in development of clinical practice guidelines/standards in support of quality care as needed.
• Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Education

Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.

Professional Experience

• Must have a Nurse Practitioner's license in the state of operations
• PMHNP certification
• Must have 1+ years experience working within Family, General Practice, Geriatric care or related field
• Experience in a nursing home, hospital, assisted living or acute rehab space is a plus but not required.
• Commitment to ongoing professional development and clinical excellence.
• Must have a valid driver's license and a reliable source of transportation
• Must be willing to travel locally
