Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Chester, PA. You’ll travel to patient homes, perform non-invasive assessments, review medical history, document findings, and provide basic wellness education.

Flexible schedule and travel reimbursement support a rewarding community-focused role. Active NP license, board certification, BLS, and a Master’s degree or commensurate experience are required.

Competenze

• Master’s Degree or commensurate experience and NP license.
• Active State NP license.
• Active National board certification.
• Current BLS certification.
• 12 months paid advanced clinical provider experience preferred.

Mansioni

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation for patient safety and compliance.
• Provide basic patient education on wellness and preventive care.

Conoscenze
