Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around York, PA. This role offers a flexible schedule and travel to patients' homes, with comprehensive training and scheduling support.

Nurse Practitioners assess patients in their homes, performing non-invasive exams and documenting medical history while ensuring safety and quality care.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Active State NP license and national board certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider preferred.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.
