Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Milton, PA. This role offers a flexible schedule and the chance to work outside a traditional clinic setting, traveling to patient homes to perform comprehensive assessments.

Qualified NPs will hold an active state license, national certification, and a current BLS credential.

Qualifications

• Masters Degree or commensurate experience and NP license.
• Active State NP license.
• National board certification.
• Current BLS certification.

Responsibilities

• Travel to patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventive healthcare measures.
