Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments for Medicare and Medicaid members in and around Philadelphia, PA. This role offers the freedom to shape your schedule while delivering non-clinical care in the patient's home, not requiring treatment or prescriptions.

Travel to patients' homes is required. Responsibilities include performing comprehensive non-invasive assessments, reviewing medical history, documenting findings, and providing basic wellness

Qualifications

• Master's Degree OR commensurate NP experience with license.
• Active State NP license.
• Active National board certification.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare.
